List of default ports used by the 3cx phonesystem:
5060 (stanard sip port) TCP/UDP
5061 (secondary sip port) TCP/UDP
5480 (portgre) (only on local machine)
5481 (apache) (only on local machine)
5482 (media server) (only on local machine)
3478 (STUN Server) TCP/UDP
7000-7500 (Default port for internall calls) (only on local machine)
9000 - 9003 (ports for external calls through VOIP Gateway) UDP
10000-20000 (RTP ports) (Used if no stun available for audio)

These ports must be opened and routed on to 3cx server.
Your second choize is to set 3cx server to a virtual server in router firewall. (if your router suports it)
Setting 3cx in DMZ is NOT recomended if your not absolutely shure what your doing.

Mori is correct except that 9000-9003 is needed for the calls, 2 ports/call. So 9000-9001, 9002-9003 would only let you have 2 calls. 9000-9009 would give you 5 simultaneous calls.
Adjust as necessary.
 
Why would all those ports need forwarding?

5060 - 5061 OK understand that.

3478 - Going to cause probs for any devices on network attempting to use external STUN servers.

9000-9xxx No point NAT will open and close

10000-20000 No point NAT will open and close

Point is, forwarding large blocks of ports, some rfc assigned and some not, is still going to cause problems on your network with other applications.
