Good afternoon,

I am getting spam calls (no answer/pickup from long number sometimes with periods) on a Yealink T42G that is remote from the 3CX pbx server. The phone rings from ext 100, 100, 1001, which we do not use. The phone is the only thing that rings, and not the softphone also on the extension.

Any thoughts?

These are most likely "rougue" or "phantom" calls. There are many people out there that constantly scan IPs and ports in the hope that they can find and exploit any and all vulnerabilities, Port 5060, as well as some other well known ports, are pretty standard and have a defined purpose, they tend to focus on these. In essence they would like to get into a system and make phone calls at your expense.

Most phones have some settings that will help to thwart such attempts as the hackers don't really know if they have reached a phone or a PBX. Look for the settings in the phone web interface such as accept calls from proxy only, require authorization, etc. The Yealink site has some suggestions on the subject.
