Took a bit to locate the pattern but we have voicemails going to everyone's Office 365 email address as attachments. The emergency line however needs to be emailed to multiple people, so I made a distribution list in Office 365 "[email protected]" and set that as the email for the emergency line ext.

No emails are being sent to it, I can email that DL just fine from any internal or external email address, I've already triple checked it's not a spam issue or Office 365 issue, the email never shows as even attempted to be delivered on Office 365 side.

We are using the 3CX SMTP, is this a known issue or something that has a workaround? Is there any other way to deliver an extensions voicemails to multiple emails?

can it be that you tried sending an email to this DL before it was created? Due to we send a bulk of messages on a daily bases, we have a very carefully set up that removes invalid email addresses (bounces) for 30 days to play nice with the big email hosters.

I did send one email to it before yes, so there is nothing to do now but wait 30 days? That seems a bit harsh.

nothing harsh about it - just careful that we maintain a high quality of email delivery with all providers.
Change the DL address and good to go...
