What devices are actually provided with a DHCP address? Could it be that your lease is expired prior making a call.
It seems (as we seem all to agree on that) that it is a nat issues. Perhaps if you are able to make all your devices behind the ADSL router fixed (or atleast all your VoIP devices) than atleast we can take that out of the loop.
DHCP although it is a nice feature, you might over complicate things by using it now.
Something different (i been playing with this atm) instead of using IP address using dns names (forward and revers dns) might be something at a later stage.
