UniFi UVP-Exec - Unable to receive INBOUND calls

UVP Phone can place outbound calls and receive internal (ext to ext) calls but the UVP phone will not ring or recognize an inbound call.

In 3CX App under Phones, I see the UniFI Phone, Type is VoIP and it properly reports the FW version but shows as "UNPROVISIONED" under the MAC address column. (see attached pic)

All configurations within 3CX are correct. i.e. UVP is in RG
