v11 403 Forbidden - Call forbidden - remote user

Hello,

We are trying to limp along this v11 for a couple months for a new client until we get everything situated for the new install of 3CX that they will hopefully approve. They have on user remote and I can't get a hold of them to view their phone web UI (if I can even get in). I have a phone successfully registered, but every call gets a call forbidden whether it's internal or external. In the PCAP, i see the server name is transmitting a "Warning: 499 (Computername) "Caller is forbidden." This extension was working fine a couple weeks ago with another phone, so I'm unsure what the issue is. I checked the obvious stuff like the Disallow use outside LAN, but since this just inexplicably stopped working, it must be something not so simple. Any thoughts are welcome!

UPDATE: Inbound calls work. Only outbound (internal and external) get the call forbidden. Outbound rules have no groups, so all users can make outbound calls.
 
It may be either an option specific to that extension number. You could test by creating a new extension. You might also try registering from a different IP. It hasn't been blacklisted has it?
Are there any other remote extensions that are working?
 
The IP is not on the blacklist. It is registering fine and inbound calls hit it perfectly. There is one other remote extension working, but the previous admin did not leave the passwords set on the phone web UI's, so we can't see those settings. However, the extension settings in 3CX are the same for both.
