Looking at my Vitelity account information, Generic SIP Support page lists both the incoming SIPxx.vitelity.net server value, and an outbound.vitelity.net (this is the outbound server, NOT a proxy.) But, intuitively I entered the outbound value into the Outbound Proxy at port 5060. Making outbound calls worked, but inbound calls I was getting a fast-busy and an error message from Vitelity.
With some help for their support, if you look at your "My Product & Services / Sub Accounts" and hover the mouse over "online" it was showing my Static IP pointing to their outbound server. It is supposed to point to the IP Address of the inbound server associated with the SIPxx.vitelity.net.
I removed the Outbound Proxy entry, and voila, inbound calls are getting in.
DO NOT SET a, Outbound Proxy value! Leave it blank.
