I have set up a new system 16.0.1.273 with several Yealink SIP-T23G and a few Yealink DECT W52P and set up the extensions to forward voicemail as an attachment. However the received wav file is only barely understandable if you down load it and amplify it 200% - On the phone it is fine.

I note this seems to be a regular in the Forums but no one seems to have cracked it. Makes this service useless. I have accessed the email from my phone, desktop, laptop etc and all are the same. This means it must be a 3CX problem in the recording.

Who are you using for the email service ?.

If third party, have you tried using 3CX Smtp Servers (making sure port 2528, is open on the firewall)

I doubt it's a SMTP issue. There are a number of threads about the volume of the recorded file. There's been no resolution that I've seen (other than cranking the volume) and it' existed for a while so I'm not sure 3CX cares enough to do anything. It's not gotten that many complaints so I figure most people listen on the phone or do transcription.

Agreed - we are using the 3CX servers to send and Office 365 to receive.

We can't use transcription due to the sensitive nature of some email.

Very sad 3CX are ignoring a glaring hole in the system - spoils an excellent product!

Please note that we have never ignored reports regarding issues with the system and although there have been some complaints in the past (in very old versions) about the recording volumes being low we did not have any reports in recent versions.
As you mentioned playing the file from the phone is OK and the volume is good. You can also navigate to the folder where the voicemails are kept and download the file manually and check the volume there.
For windows it is C:\ProgramData\3CX\Instance1\Data\Ivr\Voicemail\Extensions

You can also create a ticket with our support department so they can check your system and see what is the cause of the issue.
