What provisioning files are created from 'phone template file' and extension's Phone Provisioning and BLF tabs?

Reprovisoning an already provisioned phone should not take more than a minute. What you are describing in not normal.
When you select a device and click on the reprovision button through the management console the PBX will send a NOTIFY message to the phone asking it to check for updates.
The phone should reply with a 200 OK message and then send a few HTTP "GET" requests to the PBX requesting the various provisioning files. The PBX will reply with 200 OK to the files it can provide and send them to the phone. The time this process needs to complete depends on the size of the files. If there is a new firmware for the phone for example this process could take longer. A normal reprovision for a blf change however should not take over a minute.
The management console log will tell you which files are requested and which ones will be generated but not much more.
You will need to use wireshark to capture a reprovision and see where the delay is.

3CX has a phone template file.
The extension has 'Phone Provisioning' and BLF settings.

You mention that 3CX sends a NOTIFY request to the phone. I am guessing that the NOTIFY request triggers the phone start the provisioning process. I understand that Yealink firmware supports two auto provisioning methods. Old - without boot files and New - with boot files.

Which method does 3CX support?

The phone template file is just that...a template. The Phone provisioning/blf settings you configure on the extension configuration plug values into the template. The template contains place holders for the values in which you configure on the extension. When you save the configuration of the extension the CFG file is created.

Mostly correct. Since v15 3CX doesn't create flat configuration files (on the file system) anymore. The configuration is 'streamed' when requested. As far as the actual files, I haven't looked because i don't care but if it matters to you wireshark or even the nginx log file (you may need to increase the verbosity) will tell you.

No, this is not supported as all your configuration should come from the PBX.

I now understand that 3CX supports the old style (without boot files) and the y00...00xx.cfg (i.e. common.cfg), MAC.cfg content is generated on-the-fly and streamed to the phone. Then the phonebook and logo files are sent. I also notice that the on-the-fly files (mac.cfg and y000.cfg) do not exist in the provisioning folder.

Are the files not created because the content is streamed as needed when requested by the phone?

Wireshark will confirm the exact process and yes, as I mentioned before and as @YiannisH_3CX the file is created everytime it is requested (aka streamed). They do not existing on the file system.

Yes the files are not stored anymore. They are created and sent to the phone upon request.
