Yealink T-28 Randomly Dialing and Setting DND

I have a supervisor that did not want to give up his beloved Yealink T-28 and it has been working fine until the last month or so. At seemingly random times his phone would go into DND mode and I was convinced he was inadvertently setting it manually. The awesome CallSequitur program revealed his phone was, on its own, dialing *32 followed by dialing "0" and finally one more "0". Three phone calls are occurring on their own in very rapid succession. In the screen shot below you can see this extension dialing *32 at 22:59:25 following by two more calls of "0" at 22:59:28, all three calls within 3 seconds. When these calls were placed nobody was in the building, and it's not possible to dial that in 3 seconds.

I found the thread involving Yealink phone and custom templates dialing *32 (thanks Chris!) but I'm using stock templates. I have Yealink T-32, T-20 and this one T-28 phone is the only one experiencing this condition. I have reprovisioned it without any change. We are running version 16 update 3 (16.0.676) with everything up to date, phone using supported final firmware 2.73.0.60.

Any thoughts besides replacing the phone?

Another day and another round of mystery Yealink dialings. As shown below it started this time at 21:49:40 and extension 204 dialed " *30 ", as did extensions 207, 208 and 209. Extension 202 once again dialed " *32 " and went DND, 15 seconds after the first *30 storm. As you can see these dialings also have mystery "0" dialings. The 204, 207, 208 and 209 phones are Yealink T20P with firmware 9.73.0.60 and stock 3CX templates. Interestly the T32G Yealink phones we have seem to be immune from this.

Funnily enough (but may not be related) I have one Yealink T23G in our office which activates the loud speaker and then turns it off again. FW on that is 44.894.0.95.

I factory reset it this morning and I haven't heard it since.

is your phone used in CTI mode ? could it be some robot dialer from computer side trying to use your phone?
 
The accounts are configured to use the soft-client (default) but nobody has it loaded. On the 202 extension I changed control back to the IP phone, will see what happens tomorrow.
 
is this behavior on all your phones or only one on ext 202 ?
 
The *32 is only the 202 extension, which is my only Yealink T-28P phone. The *30 dialings are only occuring on half of my T-20P phones, less frequrently than the *32 dialer at extension 202. I do have custom template in use for my Polycom phones.
