Okay, here's the system config:
SBS 2003 running Enterprise Edition 3cx
Linksys SPA922 and 962 phones
Grandstream 4108 Gateways.
I saw this same problem posted elsewhere on the forums. However, there was no response, so here we go again, this time with a server log:
Here's the error stack

Here's what happens: I call in. I then ask to be transferred. Attended transfer begins; I hear the hold music. Receptionist finishes the attended transfer and I begin speaking to the person to whom the call was transferred. After approximately 10-20 seconds, the call terminates.
Thoughts, anybody?
