Running 3CX 18.0 (Build 418)
The firewall test passed on everything. Using Flowroute as a SIP trunk provider.

Issue: Inbound calls work fine for a while but after a certain time, when callers call in they no longer hear the IVR.
At that point the only thing that fixes it is a reboot, which is a problem if people are on calls.
 
When you say the firewall checker passed, when did you last run it? Your scenario seems to indicate you don't have all of the RTP ports open.

About a month or so ago. These are the open ports.

9000-9398UDP
10600-10998UDP

You're missing a few between 9399-10599, plus 10999 :)
5061/UDP is unnecessary.

I just opened more UDP ports once CobaltIT said it sounded like I was missing some.
So far, so good. I was trying to be as restrictive as possible in the ruleset, but it seems to be more harm than good.

Note 7000-8999 is not required/used for external connections. It would be much clearer if they made 7000 its own row in the table, but it is intended for "LAN" and port forwarding = "No..."

I thought just for funsies I'd open it and packet sniff to see if something is trying to use it.
