This sounds like either an extension setting, or a general 3CX setting variable, I would suspect the latter.
Not having access to a system at the moment I am unable to scan though all of the options.

You may want to check extension 2438 and insure that the forwarding parameters are set correctly for both internal and external calls. It sounds like you have the external call set correctly given the IVR is forwarding, but the internal may be set differently. I do not know if the conferencing will forward as I have never tried it. There is a setting in the advanced settings to allow for forwarding for ring groups and queues, but it does not indicate anything about conferencing - "ALLOWFWDTOEXTFROMGRP". If set to "1" = will allow, but again I am not sure if applicable to a conference call. I saw nothing specifically addressing the forward of a conference call.

Call from external to 2438 route to my Mobile
Call from internal to 2438 route to my mobile
Call from external to ivr to 2438 route to my mobile
Call from internal to ivr to 2438 route to my mobile

I have set allow outgoing call from ivr

I think this is or issue 3Cx or by design disable how feature

If I had to guess, this (conference call) is what I refer to as a "system owned" call. As a result it ignores the status of the extension. I suppose this could be by design as the system is then reliant on the user to have their forwarding rules set correctly and I could imagine what might happen if someone accidentally had theirs set to away and send call to voice mail; or even worse, if the system were set to change the status automatically.

Just a guess on my part.

What is this custom parameter set to?

ALLOWFWDTOEXTFROMGRP
Allows members of ring groups and queue to forward calls to external numbers. Requires that the gateway must be able to accept a call ONLY when the callee has answered - Early media OFF.
