The first thing you need to know is if your provider supports this functionality and you are allowed to send the originator caller ID. If your provider supports this then you need to know what SIP field they are using to read the number from. Then and only then you should go to your trunk outbound parameters and change that SIP field to Originator Caller ID.

When Bob calls John's Work, but John's phone is set to forward calls (or you set a DID to forward calls) to an external source (ie. John's Cell), the phone system initiates another call to John's Cell and hands Bob off to that new call. To John, the caller ID will show John's Work.
The only way to push Caller ID information is to "hard forward" a number at the telephony provider that should have a CNAM (Caller ID Name) push capability. However, this normally isn't done for temporary purposes.
If you're trying to forward to mobile phone number on a device that has Android or iOS, there is a 3CX app where you provision it as a softphone and it acts as a normal extension. This can be set on any extension as an added phone. Please visit the link(s) below:

You need one more step to make this work. You need to navigate to your trunk settings / Outbound parameters and set the sip fields that have outbound caller ID to Originator caller ID.What you might try...
Create a new extension. Assign a set temporarily. Set an outbound caller ID in the extension settings. Does that number show when calling the external number? If so, set the DID you want to use, to go to the new extension, and set call forward to the external number. Remove the outbound caller ID number, it was just for testing. Now have an external number call the DID, does their number show on the external number? If not, try the Ring my Mobile at the same time option on the extension.
