Headset hearing issues?

The phone we use keeps having issues hearing from the handset. It's a T58W phone. If we power cycle the phone, it resolves the problem. It seems to reoccur every couple of days. We pushed a firmware update to it about 2-3 weeks ago, but that wasn't a permanent solution. Any other ideas?
