The name can come from a number of sources. It can be added at the origination, sometimes by the customer (less likely), or by the originating provider. A lookup can be done from a database, or the name may have been added in the originating providers switch, against the customers number. A name lookup can be done by the terminating provider, usually if no name is attached to the call. Most providers charge for this, if a database is queried, as it is usually not owned by the provider. In some cases, the name may simply show as a city, state, province, or country, if there is no user listing found.
