Our receptionist phone is a 2140 with 2 expansion modules. Since upgrading to V16 she is unable to answer calls 100% of the time. Some calls continue to "ring" after she's picked up the receiver and she has to wait until the call forwards to the ring group to answer it.

Not sure what is going on but this is very frustrating. Anyone else have any issues with Grandstream phones?

Haven't moved to v16... seen too many bug reports.

Umm.. what does this have to do with the expansion modules? Your description just says answering calls which could be direct calls. Are you saying that she can't pickup calls on other extensions via buttons on the expansion module. Or are you saying if you disconnect the expansion modules she can answer calls without issue. If so, what flavor of 3CX, connfirm if you are using the latest 3CX supported firmware, 3CX templates, and how you are provisioning call pickup buttons on the module via the extension BLFs since that is not a direct option.

Hello,

This could also be network/server issue potentially: When the phone picks up the call, it has to send an acknowledgment back to the PBX machine, which if not received will have to consider the endpoint to still be ringing and forward the call to the group when the time is up.

Speculation is not enough of course, if you cannot resolve it through basic troubleshooting as suggested by members above then you will need to look at what is happening on the network traffic. 3CX partners can also help you with this if your background is not in IT, otherwise you can contact 3CX Support directly for further analysis.
