Multiple Phone Entries for Same Phone and Extension

I am hoping someone can assist me with an odd issue. I have recently configured our new 3CX phone system in a lab environment so we can perform testing for several of the remote branch locations before going live to confirm there are no issues with call quality and QoS.

Before I begin, we are running the latest version of 3CX 15.5.15502.6. All phones are updated with the latest 3CX supported firmware versions for the Yealink T48S (Currently 66.83.0.55). Firewall check is reporting no issues. The lab is behind a Cisco RV180 for testing only. All of our branch locations are using Fortinet firewalls. We do not have an active SIP trunk attached on the lab 3CX yet, as we are first testing internally before moving over one of our trunks. The two STUN units have PBX delivery audio checked and disallow use of extension outside the LAN unchecked.

The odd issue we are encountering is where the same phone/extension registers multiple entries (see image) but this only seems to occur over STUN. I programmed one Yealink T48S for a local LAN test phone and two Yealink T48S phones for STUN located at another branch. The local LAN phone only has one entry under phones as expected. The two STUN remote phones are showing multiple registration entries for the same phone/extension. Also, I can only make calls to and from a single STUN to the local LAN phone. Calls between STUN phones at the same location will hang until it goes straight to voicemail.

Each STUN phone is configured with its own unique local SIP port and RTP Audio Port start/end.

Ext. 252 (TestPhone 1), Local SIP Port 5066, Local RTP Audio Port 14020-14039
Ext. 237 (TestPhone 2), Local SIP Port 5067, Local RTP Audio Port 14040-14059

I have checked the event logs and I am only seeing a lot of unregistered and registered entries for these extension/device instances. I have been unable to locate anything (unless I missed it) obvious in the activity logs. We have a Fortinet firewall at each location and I am uncertain if it is possible there are any issues using those devices. We have over 15 locations and most with only a handful of users, so I would rather not set up VPNs for every site. Any advice would be greatly appreciated. Thanks!

Clearly a firewall issue. Besides creating the unique ranges at the extension side you'd also need to port forwarding in the firewall for those same ranges (which you don't mention doing). Also, as you noticed, with STUN the audio always has to go back to the PBX. I'd recommend putting a SBC at each location. It will keep extension to extension audio local and requires no firewall configuration for the brach offices (or port ranges per extension)
