Outbound calls ringing internal extension and not going out

Hi all.
I have an outbound rule on 3CX that should routes calls to a SIP trunk. Challenge is that when any extension makes an outgoing call, the calls dont go out but rings at extension 1301 no matter which outgoing number is dialed. Also when extension 1301 makes an outgoing call, it is taken to its own voicemail. Outgoing calls are not leaving 3CX and the SIP trunk has no logs for outgoing calls. Activity logs on 3CX show [Flow] Target endpoint for xxxx can not be built!.
What could be wrong with my outbound rule to cause calls to ring at an internal extension?

The initial symptom was that it was using my Proxy to make outbound calls. Now it just calls my desk no matter what I dial.

This is from out SIP provider.
Your pbx is sending to our secondary server of xxx.xxx.xxx.xxx for some reason, but it still has our primary server of xxx.xxx.xxx.xxx in the request URI. That is the reason our server is sending back that 403 message.

We didn't change anything.....
Any help is greatly appreciated.
