Polycom 331 will not reset

One of my clients is running V15 and runs about 50/50 Polycom 331/330 and yealink (varius models). We are having issues getting the Polycom 331/330 to reset to factory so we can re-provision them to a different extension. I have multiple times done the multiple key combination; hold 1, 3, 5, 7 enter MAC as password etc.. When it first restarts you can enter the setup menu from the "countdown" screen using the default password 456. However once the phone boots up it states "Unable to load bootfile loading last config" and reverts to the old settings. I have tried switching it to a VLAN that does not have DHCP 66/67 enabled and it still keeps reverting back to the previous settings. Looking to see if anyone else has come across this issue or has any recommendations. At this point I am stumped as it is happening with all of the Polycom 331/330s that we have.

Just tried resetting on of these phones and it worked as intended. My device is on firmware 4.0.10 and i followed this guide. If you are certain that the phones are not pulling configuration from any where then i would recommend contacting Polycom regarding this.
