provision from SPA508 GUI

I need to manually provision a Cisco SPA 508G phone from the web interface. my problem is :

1. I cant find my cheat sheets for this
AND
2. I don't have phone password so I cant just copy line 1 to new line 4 with correct extension parameters

I am going to have to factory reset, then rebuild my line appearances

any help for manual provision - several years ago(v11) I had same post but cannot find it
I think it is something like this?
fnc=sd+blf+cp;sub=<EXT1>@<PBX-IP>;nme=<EXT1>
fnc=sd+blf+cp;sub=<EXT2>@<PBX-IP>;nme=<EXT2>
