When one of my sites experiences an internet outage, how can I automatically redirect all calls? Or does the system redirect it to the voicemail after the queue time out?

This depends on which side of the outage your PBX is at, dial tone source and length of outage. But basically assuming 3CX is in the cloud and phones are onsite, then you will initially have agents in the queue that aren't answering, so the queue should process the same as if all agents are unavailable until it times out. Once the phones all unregister, then it will act like an empty queue. But again, without more details on your configuration this is only one of possibly many configurations/scenarios.

It’s not a cloud issue. Office network outage. All agents are in the queue and are unregistered. Since there’s no internet

By default once Call Queue Maximum Queue Wait Time 1800 sec reached, all calls will be ended from queue.

You can have option route calls to any extension's voicemail box or forward calls to external phone number or landline number.

What @cobaltit I think meant is, where is the PBX installed, on-prem or in the cloud?
If the PBX is on-prem, are you using SIP Trunks for calls or some Gateway?
