Please as first step validate that the phone is using the correct firmware 96.86.0.76 the same is available in this link
I would suggest to delete the phone from the extension, apply the changes and perform a factory reset in the phone.
Once done, please try to configure the device as router phones following these instructions
In the event log you should see a message that RPS was sent successfully. Once you see that message, then connect the phone in the network and it will take the provisioning template via RPS.
Just to mention now that I have the attention of a 3CX support person: if you didn't already see 96.86.0.76 has apparently a bug. Many people like me cannot provision a router phone with that anymore. It will provision, but then it will not register, even after total reset to factory - deleting the phone from 3CX and re-adding.
(Just as a side note: that is why I suggested to @Juangarzon to downgrade to .74, because his problem sounded very much like what I just described.)
I and many others were only able to get it working again after downgrading to 96.86.0.74.
It seems something changed in that firmware that broke the router phone function, not for all but for some Yealink T5X phones.
