Voicemails failing and routing to operator extension

Hi all,

We are having issues with one of our queues voicemails failing and then routing to our operator extension. I'm not sure how to troubleshoot this particular issue as the logs don't show any further information.

Is anyone able to provide assistance or maybe a start?
