Warning: 499 "Disabled by Forwarding Rule"

Hello,

We are using Bandwidth sip, 3cx on v 18, and Skyetel as a failover trunk.

In Bandwidth we have 3 routes for calls configured, The first two are for IP routing to the primary IP of our PBX, the secondary IP of our PBX, and a forward to a phone number on Skyetel which routes to our Primary IP on the PBX.

We are having an issue with call routing on a very few number of calls which dont seem to have a pattern so far. Every now and then, a call will be placed and will fail to connect to the first two routes (Bandwidth to PBX) then failover to the third route (Bandwidth to Skyetel via phone number to PBX). In a 24 hour period, this could happen zero - 5ish times out of hundreds of calls.

Bandwidth has stated they dont see responses from the PBX during the initial invite attempts even though I can see active calls during the time these will fail.

I was able to use one of the clients as a test but wasn't able to make it fail. For example, they called in and were routed through our failover trunk at 6:22am, then I had them test again around 9am and it went through normally.

Ive had a ticket open with 3cx but they say they cant help unless I can provide a 3cx generated support docs and wireshark. I do have wireshark running continuously on our PBX and I do have pcaps during the times of the call failures but I dont see any traffic having issues. Due to the sporadic nature of this issue I don't know when ill be able to get what they want and they keep closing my ticket.

Has anyone ever had issues similar to this? Has anyone ever seen the 'disabled by forwarding rule' in the sip invite?
