Previously setup on Ext 200.
Delete DEVICE from ext 200.
Login to Phone GUI.
Reset to Factory
Login to Phone - no account details.
Phone begins to reboot.
When done - I login to phone again.
It now has details for Ext 200.

But under Phones in Management ConsoleScreen Shot 2017-06-02 at 3.04.39 PM.png - it is a new device.

I also tried again by REMOVING the credentials from the phone... saving - then RESET to FACTORY. It still got credentials...

Also noticed - when I apply the 3cx Yealink T4x Template - 2 things happen:
1) Phone GUI is only accessible via HTTPS. Not HTTP.
2) The Dial Codes for DND on and DND off are no longer shown.

I do not think this is necessary but noticed:
static.network.dhcp.option60type
should probably be static.network.dhcp.option60type= (the = sign is missing)

Maybe you have an old config file for that extension from back when 3cx wasn't generating them on the fly ? When you delete the device from an extension, what happens if you navigate to your provisioning url / yealinkmac.cfg ?

Jason - good call. For some reason there was just 1 CFG file in the directory and it belonged to this phone.
No other handset is in there... lets see what happens now. :)

Nice. Pulled my hair out on that one once. ;)
