From what I can read, when someone sets up faxing on 3CX there can only be one incoming extension (888)? So what happens if you have 500 DID extensions and you want more than one assigned fax extension to receive outside incoming faxes and send to unique email address for a particular department or person? Is this possible?

This is possible using a fax software, whereby you setup a fax extension and the fax software logs into that extension to communicate to send and receive the fax. Inbound rules can be used to handle the DID's. Only mitigating factor would be volume and licensing, though that may not be an issue because faxs often retry, you'll have to scale for your needs.

Yes, and you don't need fax software. You simply configure the inbound rule. If you check the destination pulldown you'll see a 'Send fax to' option. Then you just pick the extension you want to send the fax to.

So once an extension is setup as a DID with fax forwarding to email that extension can no longer be used for voice correct? At least that is what I found in my testing. If you call it from outside or inside the pbx a fax machine answers.

I currently have an Avaya tied to a 3CX via sip trunk for implementation. When I send a fax from a analog desktop fax machine on the Avaya to a 3CX fax extension it answers but then never receives the fax (does not send a pdf email). Originating fax says error (37) Should this not work?
 
You absolutely can use that extension for voice (if we are talking about the normal extensions, not the FAX extensions. You just can't use that DID for voice (assuming you are using the method I mentioned)

Once you select a DID and use in bound rules to 'send fax to xxxx', it can not be used for voice. It is one or the other.
