Call Failing Over to Another User's Voicemail?

In this case, a call came through to one user but they were unable to answer it. Instead of it failing over to her voicemail, the voicemail came to the main/first user's voicemail. We do not have any ring groups set up. I was trying to dig through the Activity Logs to find out what happened, but I don't see much info in there related to this. First, what might cause one user to get voicemail from another user's call and secondly, is there anywhere I can go to track down what happened here and why?

It's worth noting, all of our users have DIDs assigned, so each number should be in their own silo, that's why I'm baffled as to how/why the call went to another user's voicemail?

Our users are all local, except when they leave the building, then they connect remotely using their phone app (iOS/Android). All ports are forwarded correctly to allow those remote users to work without a VPN.
