Call from Queue not reaching extension

Good day all,

I am hoping someone might have some insight for me.

We have multiple devisions, some of which require different recordings to provide details surrounding their specific function. The way that we have gone about this is with call queues, some of which only have one or two phones in them.

We have a SIP which is registered to one branch which rings to a queue for reception, and the reception phone is the only one in the queue.

Every now and then, it appears that the queue never connects to the phone. (By every now and then, I mean two or three times a day). One of the other branches, when phoning the landline as an external call, always gets stuck in the queue and can never reach the reception phone. The call logs and reports show the call coming into the queue, but where it would normally say "Q_R replaced by EXT 2000", it just says "Terminated by xxcallerxx" (because the caller eventually gives up holding and cuts the call).

Has anyone experienced this? If so, how did you resolve it? I'd even be grateful for where else I can look as to why these calls are getting stuck at this point.
