It's not clear that you ever got the device to even register.

Did you manage to see a registration?

Hi John, Yes the intercom is registering in 3CX.
 
Ok when you call it's extension directly it can answer the call and run for 32 seconds at least?

I can call it but the intercom won't answer because it can't.

Then make a call from the intercom to an extension, and lets ensure a call can be set up and established first. You can probably program its button to quickdial an extension

That is also not possible. is is a hardware intercom that wants to use users Smarthome and VoIP server to setup a call.

Step 1: A person presses the doorbell
Step 2: The home owner gets a notification an can view the person via a video connection.
Step 3: The home owner can now click on a Call button to setup a audio conversation.

At this point the intercom (extension 601 in this case) want to call the smartphone of the home owner or vice versa using the VoIP server.

This is my best quest of what should happen. When using a free service (iptel.org for instance), it works but we don't what this.
 
Instead of a smartphone number, can you enter an extension number? Provided it uses the voip service of the PBX it should be doable. Again, because this is not supported hardware, I cannot say whether you can eventually get it to do what you want, but you can give it a shot.

The smartphone uses a default not changeable username "[email protected]". This user wants to call the [email protected] but then we get the "408 Request Timeout/INVITE from local" error.
 
I'm not sure that what you want can even be done. The point being: you may get it to register, but calls may not be achievable in the way it wants to make them.

To make any calls using the 3CX system, you need to have two extensions BOTH registered on the system so they can call each other, or have at least one extension registered to make an external call in this order:

Device->PBX->Trunk Provider->Destination External number.

The calls will have to pass through the system SIP server either way. Again, since I don't know the device, I can't say its impossible but you may have to reach the manufacturer directly and follow their instructions to get it to run using a SIP server. You may also have to manage your own domain records to make it possible, depending on what their requirements are for Direct SIP.

To take things back to basics: Why does the device need a PBX? What does it want to achieve via the PBX that can't be done otherwise? If it wants to do something that does not need a PBX in the middle then you can skip the PBX altogether and register to your trunk provider directly.
