Before 3CX I've used freepbx with a main number and up tp 10 extensions.

e.g.
Main 49 375123456

EXT1 internal: 10 . external: 49 375123456-10

now with 3CX and wireshark I can see the same header incomming but it will not redirected to the extension, the call will still go to the main extension/queue
Code:
   54 2.904562674  2001:ab7::9 → 2a01:4f8:c2c:c5ca::1 SIP/SDP 143 Request: INVITE sip:4937512345610@[2a01:4f8:c2c:c5ca::1]:5060;rinstance=1dcafb3ec7d12ceb |

You need to create individual Inbound Rules for each DID directing to the related extension. This is not done automatically.

I made it, Thanks!.

and you use actively use IPv6 (2a01:4f8:c2c:c5ca::1)?
