Looks like they are trying to hack extensions very often, mine was hacked 2 times.
there was indeed a weird phone extension with private IP(Not my ip scope) unprovisioned
I found out that even regenerating the extension it will not work, because the windows or android app they will not need to reauthenticate, so the attacker can still have access to the extension, thats why I was hacked the second time even if I have also changed the extension password on the 1st time.
The only way is to deleted and add again the extension
