Just tried now and seems to work as expected:
- receiver (iOS client) is not running
- sender send a message - get one gray, correct, message not delivered
- receiver is launched - sender's gray become blue, correct, message has been delivered
- receiver opens the chat with sender - sender get two blue, correct, message has been delivered and read

I checked the work according to your scheme - yes, it works that way. but this behavior is not expected. the message is considered delivered not when the user opens the application, but when it arrives. The push message contains text, which means it is considered delivered upon the arrival of the push. In other systems (whatsapp and telegram), "delivered" is displayed immediately after the push arrives, without waiting for the application to open.
 
That's not correct: standard notifications are cached - therefore, when sender gets 200 OK that he sent the notification, that means it was received by the APNS server - the receiving device might be powered off or not on network. Therefore, when the receiver connects to the PBX (say, one hour later) only then message can be considered delivered.
 
if my phone is disconnected and I have not received a push message, it has not been delivered. when I turned on my phone and received a push message, it is considered delivered, regardless of whether I opened the application or not. test it - this is how it works in whatsapp.
