I have a SIP trunk dump into a ring group. When I call in, the remote caller does not hear any ringing. I switched it to just ring a phone and it wasn't any better.

The extension does ring.

Who is your trunk SIP service with? Did this issue just start?

New PBX.

We are the SIP trunk provider. It goes to our Metaswitch.

You may want to Wireshark to confirm the a 180 Ringing (or something else?) message is being sent by 3CX, if so, then it is being ignored (not acted upon), and may require a "tweak" at the other end.

So no ringing but remote audio or missing both? If both, check firewall. If just the ringing then what @leejor said

I had to set this value to false on my Metaswitch and then all was well.

PBX plays ringback
Change Inactive
Specifies whether the PBX equipment at the customer premises provides audible ringing tone to the caller on calls to a subscriber on the PBX. Values:
True
False
The default is False if the grandparent PBX object has a Signaling type of Analog line/T1 CAS, and this object's Signal function code is set to Loop start or Ground start; otherwise, the default is True.
This parameter is ignored if the PBX Line uses ISDN signaling over an ISDN Primary Rate Interface that is not configured to support ringback. The switch provides ringback unless the ISDN PRI and the PBX equipment both support it.
