Hello,

I would like to know if it's possible to change extension status based on O365 status IF extension wasn't created by O365 sync?

For example, I manually created 3cx extension 05 with email [email protected] while on O365 I already have user [email protected]. Is it possible to sync status between O365 user and 3cx extension in this case, or extension must be created with O365 sync process in order for this to work?

Yes, this is the calendar sync feature.

As long as the email in 3CX matches the principal email in 365, they will match.

Users in 3CX will only be created if you turn on the User Sync feature.

Though, what you might find if you turn on user sync is you end up with duplicates in your 3CX, so you'll have to remove the duplicates the initial sync makes.

Yes, it is still possible to sync calendar statuses for 3cx extensions from appointments created within the 365 calendars when the user's extensions are not created by using the 365 integration options for the user sync, as long as the email address of the extension matches the users 365 email address, and the user is added to the calendar sync section under the integration options within 3cx.
