This is an on-premises set-up with everything provisioned through local LAN. 3CX is the latest version (18.0 Update 3 (Build 461)) running on Win10. The specific phone I'm trying to provision is a Yealink T46S with the latest firmware (66.84.0.5). However, I have had the same problem trying another Yealink model.

I plug in the phone and factory reset it and 3CX does recognize it as a new phone. I add the phone to the extension I want, but it stays as a new phone in the list and doesn't provision. The phone does appear under "Phone provisioning" for the extension though, but when I select it and press OK, nothing happens once again and the drop-down goes back to the old phone. If I open up the phone's UI and enter in the auto-provision URL the browser page acts like it's doing something, but nothing happens on the phone itself.

I have been able to provision an extension to the 3CX Android app, and DHCP option 66 is off. Also, the certificate for our FQDN was recently renewed.

Any vlans involved in the network setup

are the phones on the same network as the 3cx server

Yes and yes. The phones, including the one I'm trying to provision, are all on the same physical and virtual network as the 3CX server

Sorry confuse by your statement ‘same physical and virtual netŵork’

please expand the setup.of the network and details of the networks where the 3cx server is on and the phones -including vlan info

Hello,

The latest firmware is actually 66.86.0.5 so please update then factory reset, then try again.
Read the guide carefully here, and you will be able to upgrade them before you try provisioning:

The phones and the 3CX server are on the same network, same subnet, and same VLAN (and nothing has changed with that for a number of years).

Oops, I thought the links right on that page were to the latest firmware. Nonetheless, I upgraded the firmware to 66.86.0.5 and factory reset, but I'm still having the same problem.

Ok Jeff lets try something and see if this helps:

  1. Check the blacklist in your dashboard, ensure that no local IPs have been banned
  2. Edit the extension that has one of the Yealinks, and delete all the entries in Provisioning so that only "3CX app" is left.
  3. Press OK to save, and then edit the same extension again, double-checking this time that the list remained empty.
  4. Now go to the main "Phones" page, and delete the bold entry for that specific phone using the Reject button.
  5. Factory reset the phone, and when it comes up, reload your phones page. You should see it appearing as "new" again
  6. Select the Phone, and this time press "Add Ext" which will assign it to a brand new extension
  7. DO NOT change anything at all in this extension, simply press OK to complete the provisioning
Using this method, we will make sure that the phone receives default provisioning settings, and if this test succeeds we might be onto something.
