I have downloaded and installed the on-premise windows VM 3CX trial standard annual 16 SC v15.5.5602.6.
I understand that the maximum number of simultaneous conversations is limited to 16.
Is there a limit to the number of extensions that can be added?
Is there a limit to the number of gateways (i.e. SIP trunk - Add Gateway - Generic)?
During the process of registering a gateway, is one of the simultaneous conversations used?
Does a gateway that remains registered use up one of the simultaneous conversations?

When an extension makes a call, is there a 'conversation' used between the phone and the 3CX PBX server?
Then is there a 'conversation' used between the PBX and the gateway?
Does one call use two SCs?

No limits other than the simultaneous calls. SC is only consumed for active calls. In most all scenarios an active call only consumes 1 SC. Some scenarious such as forwarding an incoming call out to a cell phone consumes 2.

You are only limited by the extension length you choose and the machine the PBX is running on.

An outbound call only uses one sim call. Also an extension to extension call only uses 1 sim call. With a couple of exceptions as @cobaltit mentioned all active calls use 1 sim call.
