Forwarding Rules Issue - Multiple Trunks

We are having a forwarding rule issue with multiple trunks, and I wondered if anyone had ever encountered this before.

Scenario:
Extensions 0001 - 0005 are on Trunk 1 and Extension 0006 is on Trunk 2
Extension 0001 has a lunch forwarding rule where it rings for a second and then forwards to an external number (cell phone). This is set up for internal and external calls.
When Extension 0001 is in "Lunch" status, that forwarding rule works perfectly for Extensions 0002 - 0005 and all external calls but not Extension 0006. When Extension 0006 dials Extension 0001, it simply gets one ring and then it ends the call with the message "Call failed, Not found".
When Extension 0001 is "Available" all calls from Extensions 0002 - 0006 work correctly, no problem.

Look at the activity log to see what is going on.

I would guess the call is failing, as extension 006 is not allowed to use trunk 1 or ext 001 not allowed to use trunk 2, the activity log will confirm this.

You probably need to adjust your outbound rules
 
Okay, so I will profess myself to be a novice at this stuff, so please forgive the ignorance. Here is what I see in the activity log (Numbers changed to hide IP addresses and match the scenario laid out above):

01/30/2019 10:33:38 AM - [CM503003]: Call(C:754): Call to "Outbound Caller ID of Ext. 0006" <sip:Cell Number@IP Address 3:5060> has failed; Cause: 404 Not Found/INVITE from IP Address 3:5060

01/30/2019 10:33:38 AM - [CM503003]: Call(C:754): Call to <sip:Ext. 0001@IP Address 1:5060> has failed; Cause: 487 Request Terminated/INVITE from IP Address 2:60783

Given what you said above, does this match what you expected to see in that I need to adjust my Outbound Rules?

I'm amusing that "IP Address" is that of your provider. That being the case, they don't appear to like the number being sent. You can compare the number sent in the logs you posted, with a working call, what is different?
 
Comparing the call that worked (from Trunk 1) to the call that did not work (from Trunk 2):

The first (bottom) line is there and identical, I assume because the call did not go through to the initial intended recipient, Ext. 0001.

However the second and third (top two) lines are not there at all, I am assuming because the call connected with the mobile number just fine which implies something with IP Address 3 if I go off of what you said.

Does this mean I need to speak with my provider on this? If so, any advice as to what to say specifically? (Again, novice.)
